You get credit or blame for the outcome

Not if you can find someone to blame

Yes, that is why you should never take responsibility

2. If a worker saw a hazard but didn't bother to report it, would he be considered irresponsible?

Not if it wasn't in his job description

No, unless he know who to blame

Yes, because it is only common decency to report hazards

3. Who should you blame, if you make a mistake or cause a problem?

Blame someone no one likes, so you won't be punished

If you have character, you take responsibility for your actions

Blame your parents for not being nurturing enough
