A major motivation for doing well in school comes from having some dreams or plans for what you want to do after you are through with school. Those dreams will give you the desire, drive and motivation to excel in school, because you are preparing for the work you would love to do.
One problem many students who don't do well in school have is that they have no idea what they want to do when they get out of school. When they do get jobs, it may be work that they don't like.
The best way to start getting motivated is to make a list of all the things you enjoy doing. Then try to think of some job you would like to get where you would be doing the type of things you love to do. Also consider the things you would like to get or buy as a result of that job. These would be your goals that you want to achieve.
Try to take classes that relate to your dream job. You can be then motivated. Getting good grades in those classes and in other ones are necessary to become someone special in your area of interest.

Answer
One thing to do is to formally set some realistic goals of the grades you want to get in each class. Write down those grades and post them where you can see them, as a reminder. This can help you focus.
Also, setting a good study schedule that allows you to have fun time as a reward for completing your assignments is good to avoid slacking off.
Since you want to improve, you have taken the first big step on being able to work up to your abilities.
